ITEMS NEEDED FOR YOUR BANKRUPTCY APPOINTMENT:
  1. Bank statements for the last six (6) months.
  2. A copy of the recorded deed and mortgage to all real estate.
  3. A copy of the title to all motor vehicles, mobile homes, trailers and boats.
  4. Copies of the debtor's last state and federal income tax returns, including all 1099 and W-2 forms.
  5. The last six (6) months of payment stubs.
  6. Copies of all notes, loan disclosure statements and other documents relating to loan transactions to which the debtor is a party, such as for cars, houses, etc.
  7. Copies of utility bills & evidence of expenses, such as receipts, etc.
